Show answer and explanation

Correct answer: (C) is, are

Explanation

The subject "this book" is singular, so "is" is correct for the first blank, even with the phrase "including all its chapters". In "the students as well as the instructor", the verb agrees with the plural "students", so "are" fits the second blank.
